Whatever mode you select (keyboard, mouse, etc.) when the game as started, you have to press the required input when the button lights up green.
In keyboard and gamepad modes, don't click on the buttons, you actually have to press the right key (arrows) or button (face) when the image is green. Be careful, it's pretty easy to lose lives.
